- your personal data will be processed for the provision of medical services, based on art. 6 para. 1c of the general data protection regulation of 27 April 2016 and art. 9 para. 1h of the general data protection regulation of 27 April 2016;
- only entities entitled to obtain personal information pursuant to the binding legal provisions will receive your information;
- your personal information will be stored for 20 years;
- you have the right to demand that the Controller provide access to your personal data, correct it or limit its processing;
- you have the right to file a complaint to a supervisory body;
- providing your personal data is obligatory, based on the legal provisions
